As a computer professional with over 25 years of experience in health care, I spent months moving ECLIPSE through the maze to get it ONC-ATCB certified as a Complete EHR<\/strong> with CCHIT. Yesterday, a doctor told us that \u2013 when it comes to Medication Reconciliation (170.302j)<\/strong> \u2013 he\u2019d rather take the word of all the \u201cexperts\u201d he\u2019s seen at seminars who indicated that he must<\/em> perform medication reconciliation for all his patients.<\/p>\n

So let\u2019s examine the facts. First of all, once you own certified technology, the only things you\u2019re responsible for reporting are covered in 170.302n \u2013 Automate measure calculation <\/strong>and 170.304j\u00a0\u2013 Calculate and Submit Clinical Quality Measures<\/strong>. You may report on a subset of these \u2013 depending on your degree and scope of practice.<\/p>\n

Next, let\u2019s examine Appendix A<\/strong> from the CCHIT Automate measure calculation<\/strong>
\ntest script. Here\u2019s the only section that references medication reconciliation:<\/p>\n

\u201cThe EP, eligible hospital or CAH who receives a patient from another setting of care or provider of care or believes an encounter is relevant should perform medication reconciliation\u201d<\/p><\/blockquote>\n
